Toothache Emergency Sydney
What's causing your
toothache?

A toothache is your body’s signal that something is wrong and needs attention. The causes range from a cracked tooth or lost filling to pulp infection or a dental abscess — and the severity of your pain often reflects the severity of the underlying cause.

Severe, throbbing toothache that is worse when lying down, keeps you awake, or is accompanied by swelling is typically caused by pulp infection or an abscess — situations that require same-day treatment. Even moderate, persistent pain that has lasted more than 24–48 hours warrants an assessment.

Toothache with facial swelling = call immediately

If your toothache is accompanied by facial swelling, difficulty swallowing, fever, or swollen lymph nodes in the neck, this may indicate a spreading dental infection. Call 02 9221 1685 immediately — or proceed directly to Sydney Hospital Emergency if outside business hours.

First Aid Guidance
What to do
right now.
Do
Don't
Take over-the-counter pain relief (ibuprofen if no contraindications, paracetamol otherwise)
Apply heat to the face — this increases blood flow and can worsen the infection
Apply a cold pack to the outside of your face for 15-minute intervals to reduce inflammation
Apply aspirin directly to the gum or tooth — this causes chemical burns to the soft tissue
Call Horizon Dental immediately — the sooner you call, the better the available appointments
Ignore a toothache that has lasted more than 48 hours — it will not resolve without treatment
Keep the area clean — gently rinse with warm salt water to reduce bacterial load
Self-medicate with antibiotics not prescribed to you — this masks symptoms without treating the cause
Note when the pain started, what makes it worse, and whether there is swelling
Delay calling if you have swelling, fever, or difficulty swallowing — these are serious warning signs

When is a toothache a dental emergency?

A toothache becomes an emergency when it is severe, throbbing, or affecting your ability to sleep or eat; when it is accompanied by facial swelling, fever, or swollen neck glands; when there is visible swelling in the gum; or when pain persists for more than 48 hours. Any of these warrant same-day care.

Mild sensitivity can resolve on its own. However, severe or persistent toothache almost never resolves without treatment — it typically worsens. Dental infections don’t self-cure; they grow and spread. A toothache that briefly stops hurting is not a sign it has healed — it may mean the nerve has died, which requires root canal or extraction.

Not necessarily. The treatment depends on the cause and severity. Mild toothache from decay may only need a filling. If the nerve is infected, root canal therapy is typically required. In some cases, extraction may be the most appropriate option. Dr Kozor will assess and explain the best options at your appointment.

Ibuprofen (Nurofen) 400mg every 6–8 hours with food is generally more effective for dental pain than paracetamol because it has anti-inflammatory properties. If you cannot take ibuprofen (stomach issues, asthma, blood thinners), paracetamol at full dose is appropriate. Topical numbing gels (Bonjela) provide minimal, short-term relief.
