All-on-4 uses four implants per jaw — two placed vertically at the front, and two placed at an angle of up to 45° at the rear — to distribute bite forces across the arch. This angled placement allows implants to be positioned in areas where bone density is higher, avoiding the major sinuses and nerves that often limit conventional implant
placement.
The technique was developed to provide a fixed, immediately loadable full-arch restoration for patients who have lost most or all of their teeth, or who face full arch extraction. A full set of fixed provisional teeth can often be fitted on the same day as surgery, with the final porcelain restoration placed after full healing.
